Concept art is a way to convey the mood or visual representation of the final product for films and video games. It is however not only used for these, It may be used in car design or arcitectual design. Concept art takes many forms like photo realistic or other more traditional techniques such as pen and paper. Generally artists will produce a large amount of sketches in the early stages to get a broad range of ideas.

Assassins Creed

Game developers then use the pictures that have been selected as the best, These would be of weapons, vehicles and environment. Don't be alustioned that concept art is just rough sketches slapped on to some scraps of paper. Just take a look at the standard of this concept drawing for Assassins Creed. The detail is amazing and really show the off how the environment should not only look but feel, It gives the sense that this is a real place.
